Office Information: About Your Visit
Insurance Information
We accept most major HMO, POS, and PPO insurance plans as well as Medicare and Medicare replacement plans. Should you have a compensable work related injury/illness we will be happy to work with your adjustor or Nurse Case Manager and file your claim once approved.
Appointments
Please call the office and make an appointment in advance. Most appointments are scheduled within 24 hours barring normal holiday closures. Should your insurance company require a Primary Care Physician referral/authorization, please bring it with you at the time of service. If you are unable to keep your appointment please be courteous and call as far in advance as possible so we may use that time to see another patient in need of medical care. We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we are late due to emergencies or hospital surgery.
Minors
Minors, age 17 and under, must be accompanied by a parent or guardian during each visit.
Fees & Payments
It is the medical community as well as the patient's responsibility to make every effort to decrease the cost of medical care. Therefore, we request payment for all services at the time they are rendered. If we are a participating provider of your insurance company, we will file a claim on your behalf. Co pays and deductibles are the patient's responsibility and will be collected at the time of service. We accept cash, personal check, MasterCard, Visa, Discover and American Express for your convenience. Remember, your insurance coverage is a contract between YOU and YOUR insurance company. We will be happy to assist you in any way that we can in handling claim issues. We utilize a recovery service for delinquent accounts unless prior payment arrangements have been made.
Prescriptions & Renewals
Please request all prescriptions and authorizations for renewal between 9:00am – 5:00pm Monday to Friday, when our full records are available. Renewals requested at other times will be filled only for extreme or emergent circumstances.
